Job summary

Evolve Therapy Services, LLC in Waterbury, CT is seeking a licensed Occupational Therapist for an in-house skilled nursing facility. You will plan, organize, and direct OT services to ensure high-quality resident care while complying with federal, state, and local standards.

Responsibilities include screenings, developing treatment plans with goals, coordinating with families and the interdisciplinary team, scheduling therapy sessions, documenting and billing, and supervising COTA as needed to

Qualifications

• Degree in Occupational Therapy.
• Active Occupational Therapy license in the practicing state(s).
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention for each patient.
• Evaluate residents and develop treatment plans with short- and long-term goals.
• Interpret and communicate evaluation findings to residents, families, and the interdisciplinary team.
• Establish individual resident therapy schedules with the team.
• Conduct therapy per plan and ensure proper documentation and billing.
• Perform other duties and supervise COTA as needed with compliant practices.

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

Full Time, Part Time, or PRN positions available

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
