Job summary

Evolve-Therapy-Services seeks an Occupational Therapist for an in-house Skilled Nursing Facility, serving as-needed (PRN). You will plan and direct OT services to maintain high-quality resident care, following federal, state, and local standards.

Qualifications

• Degree in Occupational Therapy required.
• Active Occupational Therapy license in practicing state(s) is mandatory.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best intervention or treatment.
• Evaluate residents to inform treatment plans and establish short- and long-term goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned to effectively treat patients.
• Oversee COTA as needed and ensure documentation, treatments, and reimbursement practices are followed with clinical excellence.

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:
