Evolve Therapy Services in Fremont, OH, seeks a compassionate Occupational Therapist for an in-house Skilled Nursing Facility. The role focuses on planning, organizing, developing, and directing OT services to ensure the highest quality resident care.

You will screen residents, develop treatment plans, communicate findings to families and the team, set schedules, document and bill OT services, and supervise COTA as needed. Active OT license and ongoing CE are required.

Qualifications

• Degree in Occupational Therapy
• Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education to maintain licensure and certification

Responsabilités

• Conduct regular screening of residents to determine best intervention or treatment for each patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to treatment plan and ensure proper documentation and billing of OT services
• Perform all other duties as assigned to effectively treat patients
• Oversee COTA as needed and ensure documentation and treatments are carried out with clinical excellence and proper reimbursement practices

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient

• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals

• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members

Education & Qualifications:
