Job summary

@Home Therapy is seeking a licensed Occupational Therapist (OT) to provide home-based occupational therapy for adult and geriatric patients throughout Queens. You will conduct home visits, assess function, and develop individualized treatment plans to maximize independence in patients' own homes.

This independent contractor role offers flexible scheduling, control over caseload, dedicated scheduling support, and one-on-one patient care with competitive per-visit compensation.

Qualifications

• Current New York State Occupational Therapist license.
• Strong communication and clinical reasoning skills.
• Comfortable working independently and using EMR documentation.

Responsibilities

• Perform occupational therapy evaluations in the home.
• Assess ADLs, IADLs, upper body function, cognition, home safety, and independence.
• Develop treatment plans to improve daily function and quality of life.
• Train patients and caregivers in adaptive equipment, energy conservation, fall prevention, and safe performance of activities.
• Monitor progress and modify plans as needed.
• Complete timely EMR documentation and collaborate with physicians.
