Job summary

VitalCaring Group is seeking a Home Health Occupational Therapist (OT) for a full-time, field-based role. You will evaluate patients at home, develop individualized plans, and deliver interventions to restore function and independence.

You will educate families, coordinate with clinicians, and participate in discharge planning while supervising assistants as needed. A valid OT license, NBCOT, CPR, and driver’s license are required.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• NBCOT certification and current state OT license.
• Minimum of one year clinical experience; home health preferred.
• Current CPR certification and valid driver’s license.

Responsibilities

• Evaluate patients' functional status and home safety.
• Develop individualized treatment plans to improve independence.
• Provide therapeutic interventions to restore function.
• Educate patients and families on treatment plans and safety.
• Coordinate with interdisciplinary team and participate in discharge planning.
• Supervise Certified Occupational Therapy Assistants as required by state policy.
• Oversee home health aide care when therapy is the sole skilled service.

What You’ll Do

As a Home Health Occupational Therapist , you will play a critical role in helping patients regain independence, improve daily function, and safely navigate life at home. You’ll partner closely with patients, families, and interdisciplinary teams to deliver personalized, high-impact care.

Patient Care & Clinical Impact

• Evaluate patients’ functional status, including mobility, strength, coordination, communication, and ability to perform daily activities
• Assess home environments to identify safety risks and recommend modifications or adaptive equipment
• Develop individualized treatment plans with clear, measurable goals focused on improving independence and quality of life
• Provide therapeutic interventions to restore function, including retraining in daily living activities
• Fabricate splints and train patients in the use of adaptive, prosthetic, and orthotic devices as appropriate
• Support patients returning to work through functional and work capacity evaluations
• Educate patients, families, and caregivers on treatment plans, safety, and ongoing care needs
• Collaborate with the care team to coordinate services and ensure seamless, patient-centered care
• Participate in discharge planning to support successful transitions and continued progress

Care Coordination & Communication

• Document patient care accurately and timely within the electronic medical record (EMR)
• Communicate plans of care with physicians and interdisciplinary team members
• Participate in case conferences, team meetings, and ongoing clinical collaboration
• Maintain strong, professional relationships with patients, families, and care team members

Leadership & Oversight

• Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations and company policy
• When therapy is the sole skilled service, provide oversight of home health aide care as required

Requirements

• Graduate of an accredited Occupational Therapy program
• Current registration with the National Board for Certification in Occupational Therapy (NBCOT)
• Current state licensure as an Occupational Therapist
• Minimum of one (1) year of clinical experience preferred (home health experience a plus)
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto insurance
