VitalCaring Group is seeking a dedicated Occupational Therapist for home health to help patients regain independence and improve daily function in their own homes. You will partner with families and interdisciplinary teams to deliver personalized, high-impact care.

You will evaluate functional status, modify home environments, and create measurable treatment plans while supervising COTAs and coordinating discharge planning to ensure seamless, patient-centered care.

Formación

• Graduate of an accredited Occupational Therapy program.
• Current NBCOT certification and state OT licensure.
• Minimum 1 year of clinical experience preferred, home health a plus.
• Current CPR certification and valid driver’s license with reliable transportation.

Responsabilidades

• Evaluate patients’ functional status, including mobility, strength and daily activities.
• Assess home environments to identify safety risks and suggest modifications.
• Develop individualized treatment plans with measurable goals to improve independence.
• Provide therapeutic interventions to restore function and retrain daily living activities.
• Supervise COTAs per state regulations and oversee home health aide care when required.
• Document care in the EMR and communicate plans with physicians and the care team.
• Coordinate services and participate in discharge planning for smooth transitions.

What You’ll Do

As a Home Health Occupational Therapist , you will play a critical role in helping patients regain independence, improve daily function, and safely navigate life at home. You’ll partner closely with patients, families, and interdisciplinary teams to deliver personalized, high-impact care.

Patient Care & Clinical Impact

• Evaluate patients’ functional status, including mobility, strength, coordination, communication, and ability to perform daily activities
• Assess home environments to identify safety risks and recommend modifications or adaptive equipment
• Develop individualized treatment plans with clear, measurable goals focused on improving independence and quality of life
• Provide therapeutic interventions to restore function, including retraining in daily living activities
• Fabricate splints and train patients in the use of adaptive, prosthetic, and orthotic devices as appropriate
• Support patients returning to work through functional and work capacity evaluations
• Educate patients, families, and caregivers on treatment plans, safety, and ongoing care needs
• Collaborate with the care team to coordinate services and ensure seamless, patient-centered care
• Participate in discharge planning to support successful transitions and continued progress

Care Coordination & Communication

• Document patient care accurately and timely within the electronic medical record (EMR)
• Communicate plans of care with physicians and interdisciplinary team members
• Participate in case conferences, team meetings, and ongoing clinical collaboration
• Maintain strong, professional relationships with patients, families, and care team members

Leadership & Oversight

• Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations and company policy
• When therapy is the sole skilled service, provide oversight of home health aide care as required

Requirements

• Graduate of an accredited Occupational Therapy program
• Current registration with the National Board for Certification in Occupational Therapy (NBCOT)
• Current state licensure as an Occupational Therapist
• Minimum of one (1) year of clinical experience preferred (home health experience a plus)
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto insurance
