Job summary

ABL Health Care is hiring a Full-Time Occupational Therapist (OT) to join our Home Health team in Discovery Bay, CA. You will assess patients, develop care plans, and deliver skilled therapy in patients' homes.

Join a collaborative team led by the Clinical Director. Travel within a 15–25 mile radius, using company devices for documentation and communication. A CA license, Home Health or Hospice experience, and a valid driver’s license are required.

• Current CA Driver's License
• Able to travel within a 15- to 25-mile radius

Responsibilities

• Provide skilled intermittent OT services in patients' homes
• Assess patient health, develop and implement POC
• Regularly evaluate and re-evaluate patient status
• Document care using company devices and maintain records
• Coordinate with Clinical Director and field staff
• Educate patient/family/caregivers on condition and care strategies
• Communicate with other disciplines as needed
• Ensure compliance with company policies and care standards

Position Overview

We are seeking an experienced, self-driven Full-Time Occupational Therapist (OT) to join our dedicated Home Health team and provide exceptional care throughout the region. As an Occupational Therapist (OT), you will evaluate patients' health, create and implement care plans, and track their progress to maintain the highest standards of care. You'll be working closely with our Clinical Director and Clinical Staff, providing support both remotely and independently in the field.

Responsibilities

• Provide, coordinate and oversee skilled intermittent care to patients and include instruction and education of patients and/or caregivers to promote maximal functional status, in the patient's home.
• Regularly evaluates and re-evaluates patient status.
• Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or as needed Director of Nursing.
• Initiates, develops, implements and revises the patient's plan of care (POC).
• Addresses all problems identified in the POC.
• Provides educational information to the patient/family/caregiver concerning knowledge deficits or needs regarding the patient's condition.
• Communicates with other disciplines involved in the case as warranted.
• Maintain accurate clinical records and submit documentation according to company policy using a company-provided device.
• Comply with all company policies and procedures to ensure high standards of care.

Requirements

• Current OT License to practice in California - Required

• Able to travel within a 15 - 25 mile radius on a regular basis.
