Job summary

ABL Health Care in California is seeking a compassionate Occupational Therapist (OT) for our Home Health team in San Pablo. This full-time role focuses on delivering high-quality in-home care to patients, assessing needs, and developing practical treatment plans.

You will work with our Clinical Director and staff, performing evaluations, creating and adjusting care plans, and coordinating care with caregivers and physicians. A travel-ready license and strong communication skills are essential.

Qualifications

• California OT license required.
• 1 year experience in home health or hospice.
• Valid California driver’s license.
• Willingness to travel 15–25 miles regularly.

Responsibilities

• Provide intermittent skilled OT care in patients' homes.
• Evaluate patients and develop care plans with clinicians.
• Document care and progress using company devices.
• Collaborate with Clinical Director and team remotely and in the field.
• Educate patients/caregivers on condition and home therapies.
• Maintain accurate records per policy.

Position Overview

We are seeking an experienced, self-driven Full-Time Occupational Therapist (OT) to join our dedicated Home Health team and provide exceptional care throughout the region. As an Occupational Therapist (OT), you will evaluate patients' health, create and implement care plans, and track their progress to maintain the highest standards of care. You'll be working closely with our Clinical Director and Clinical Staff, providing support both remotely and independently in the field.

Responsibilities

• Provide, coordinate and oversee skilled intermittent care to patients and include instruction and education of patients and/or caregivers to promote maximal functional status, in the patient's home.
• Regularly evaluates and re-evaluates patient status.
• Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or as needed Director of Nursing.
• Initiates, develops, implements and revises the patient's plan of care (POC).
• Addresses all problems identified in the POC.
• Provides educational information to the patient/family/caregiver concerning knowledge deficits or needs regarding the patient's condition.
• Communicates with other disciplines involved in the case as warranted.
• Maintain accurate clinical records and submit documentation according to company policy using a company-provided device.
• Comply with all company policies and procedures to ensure high standards of care.

Requirements

• Current OT License to practice in California - Required
• Experience in Home Health or Hospice care - 1 Year Required
• Current CA Driver's License
• Able to travel within a 15 - 25 mile radius on a regular basis.
