Occupational Therapist OT - Home Health Services - Full Time Available

Our Compensation Plan for our clinicians is top of market and pays higher for visits that require greater effort. By assigning higher point values, we can directly tie your compensation to the work you perform, and time spent outside of a visit.

JOB PURPOSE:

Provide evaluative, consultative, and treatment interventions to all home health patients referred by a licensed physician in their place of residence.

KEY RESPONSIBILITIES:

  1. Assesses, monitors, documents and reports progress of patient's health and condition using required documents via clinician preferred technology, and within established timelines.
  2. Helps maintain continuity of care by collaborating with the interdisciplinary team and attending physician to develop, integrate, and manage the patient care plan of care, making referrals to other disciplines as needed.
  3. Demonstrates intermediate knowledge of home care and competency in discipline for specific patient care skills, required for the provision of care.
  4. Recognize and respond to priorities in patient care with initiative and sound judgment.
  5. Assist with training/orienting others.
  6. Utilizes accepted company teaching materials in patient/family education.
  7. Completes documentation timely, accurately, and at the point of care, according to industry standards (e.g., OASIS assessments, SOC/Admissions, Recertifications, ABN’s, subsequent visits, physician orders, care coordination, etc.).
  8. Report any alleged or suspected violations to supervisor and/or others until the issue is resolved.
  9. Complies with corporate compliance program.
  10. Meets agency productivity standards and utilizes time and resources effectively and efficiently.
  11. Participates in center/agency surveys (Licensure/Joint Commission), mandatory in-services, case conferences, staff meetings, and programs (e.g., Performance Improvement Program) as required.
  12. Follow established safety regulations and procedures, and report job-related functions/tasks that involve occupational hazards, as necessary.
  13. Maintains current license, certification, and clinical records to meet federal, state and agency regulations and guidelines.
  14. Ability to be self-directed, prioritize, manage time appropriately, and meet outcomes and strategic goals by targeted deadlines.

Qualifications

LICENSURE, CERTIFICATION AND EDUCATION REQUIREMENTS:

  1. Must have a valid and unrestricted professional license in state of practice.
  2. Graduate of ACOTE accredited school.
  3. If foreign-trained, the program must be deemed substantially equivalent to an Occupational Therapist entry level education in the U.S. by a credentials evaluation organization approved by the AOTA.

PREFERRED SKILLS AND EXPERIENCE:

  1. Prior Home Health experience is preferable.
