Qualifications

Location: This position involves patient visits in the Hillsborough County Area.

The role you’ll contribute:

Executes physician's orders and notifies the physician of necessary updates or changes in a home health patient's condition. Assesses, plans, implements, and evaluates the individualized occupational therapy care provided to a patient. Responsible for the coordination of care for all assigned patients. Delivers high-quality and professional occupational therapy care. Continued employment depends on your ability to perform the outlined job duties.

The value you’ll bring to the team:

  • Comply with all department policies, procedures, practices, and standards of care.
  • Perform comprehensive initial evaluations on all assigned patients requiring occupational therapy.
  • Conduct comprehensive home safety evaluations related to activities of daily living and communicate specific needs to the physician and team members.
  • Establish a care plan with goals based on the patient’s current functioning and potential for improvement, in collaboration with the patient, family, and caregiver, and in accordance with physician’s orders.
  • Implement the occupational therapy assessment and care plan, continuously evaluating the patient’s response and adjusting as needed.
  • Report pertinent observations to the attending physician and modify the care plan as directed.

The expertise and experiences you’ll need to succeed:

Minimum qualifications:

  • Graduate of an occupational therapy program approved by the Council of Medical Education of the A.M.A. in collaboration with the American Occupational Therapy Association or equivalent.
  • Registered by the American Occupational Therapy Association.
  • At least 1 year of clinical experience as an Occupational Therapist in an acute or rehab setting.
  • Occupational Therapist – Registered within the state of FL.
  • Current BLS certification from the American Heart Association.
  • Valid driver’s license with current car insurance in the state.

Preferred qualifications:

  • 2+ years of home health occupational therapy experience.
