Job Summary:

The Occupational Therapist (OT) will provide coordinated care to clients of all ages. Responsibilities include planning, implementing, and evaluating client care plans to restore or maintain well-being, assessing occupational roles, and providing interventions for physical, psycho-social, and cognitive impairments.

Job Responsibilities:

  • Promote and exemplify the company's mission, vision, and values.
  • Perform clinical assessments to identify patient needs and appropriateness of care.
  • Document, prepare, and revise Plans of Care (POC), progress notes, and other clinical documentation.
  • Apply infection control and universal precautions to protect clients and staff.
  • Assess and facilitate clients' ROM, strength, coordination, adaptive equipment use, environmental modifications, functional mobility, and safety related to ADL performance.
  • Communicate effectively with clients, families, team members, and other healthcare professionals, participating in case conferences and team meetings.
  • Evaluate clients' occupational performance in self-care, work, leisure, and rest, considering sensory-motor, psycho-social, and cognitive-perceptual components.
  • Monitor cases for compliance with third-party payers' requirements.
  • Complete documentation promptly to ensure compliance with policies.
  • Participate in in-service programs and maintain professional competency.
  • Meet mandatory continuing education requirements.

Requirements:

  • At least 2 years of experience as an OT in a clinical setting.
  • Preferred: Home health experience.

Home Health OT Requirements:

  • Current State professional license as an OT in applicable states.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and insurance.
  • Skills aligned with accepted standards of practice.
  • Excellent observational, problem-solving, decision-making, and report-writing skills.
  • Strong interpersonal and communication skills.

Education:

Bachelor of Science in Occupational Therapy from an APTA-approved program.
