Job summary

Interim HealthCare of Augusta is seeking a Home Health Occupational Therapist (OT) to deliver skilled therapy in patients' homes within Lexington County, SC, and surrounding areas. You will assess, plan, and implement therapy to help patients regain independence and prevent hospitalizations, collaborating with a home health team for coordinated care.

The role emphasizes one-on-one patient care, flexible scheduling, and ongoing professional development, including free CEUs and tuition assistance,

Qualifications

• Active Occupational Therapy license.
• Graduate of an accredited Occupational Therapy Program.
• Successful completion of the NBCOT certification examination.

Responsibilities

• Provide occupational therapy to patients unable to perform daily tasks due to illness or injury.
• Work as part of a home health team including RN, LPN, CNA, HHA, PT and SLP to implement the plan of care.
• Assess patient, observe deficits, establish therapy goals and document progress.
• Assist patient with exercises to improve fine motor skills and coordination.
• Suggest adaptive equipment such as grab bars and shower chairs to improve safety.
• Assess fall risks and introduce home safety strategies.
• Educate patient and family on plan of care, exercises, goals and self-care.

What You’ll Need
