Job description

This position is for an Independent Contractor to serve the Rohnert Park area specifically.

FeldCare Connects is currently seeking a self-motivated Occupational Therapist to deliver excellent care and work with adults of all ages.

The Occupational Therapist in Home Health is responsible for assessing and evaluating patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, adaptive equipment, and other occupational therapy needs as defined by medical conditions. Based on this assessment, the Occupational Therapist develops a treatment plan, performs interventions to improve the patient's wellbeing, and evaluates progress.

Responsibilities include:

• Providing services according to state laws and the plan of care, in coordination with the healthcare team.
• Leading patients in therapeutic, creative, and self-care activities to enhance functioning and safety.
• Observing, recording, and reporting patient responses and condition changes.
• Instructing patients, families, caregivers, and the healthcare team in therapy participation.

Qualifications:

• Enthusiasm and ability to work with all ages.
• Occupational Therapist license and registration by the state.
• Completion of an accredited Occupational Therapist program.
• Bilingual skills are a plus!
