Job summary

HealthPro Heritage and its Masonicare facility in Wallingford, CT seek a compassionate Occupational Therapist to support both inpatient and outpatient SNF settings. The role is full-time with a split between our Masonicare and Mary Wade facilities, focusing on meaningful patient progress.

You will assess, treat, educate, and document plans, collaborate with an interdisciplinary team, and contribute to growth and mentorship within a supportive, mission-driven environment.

Qualifications

• Education: Degree in Occupational Therapy from an accredited institution.
• Licensure: Valid state licensure as an Occupational Therapist, or license eligible.
• Skills: Strong clinical assessment and diagnostic skills. Proficient in developing and implementing effective treatment plans. Excellent communication and interpersonal skills with a patient-centered approach.
• Physical Requirements: Ability to lift and assist patients as needed. Stand for extended periods.

Responsibilities

• Patient Assessment: Evaluate clients' physical, emotional, and cognitive abilities using observations and standardized tools.
• Treatment and Education: Create personalized treatment plans and educate clients and families on improving daily activities.
• Documentation: Maintain accurate records of evaluations, progress, and treatment plans.
