Job summary

Catholic Health Home Care in New York seeks a licensed occupational therapist to provide and manage home health OT services across our Long Island service area.

You will assess, plan, and implement patient-centered care, coordinate with other disciplines, and communicate status changes to the care team while upholding professional standards and growing within a people-first healthcare system.

Qualifications

• 2 years of experience as an occupational therapist.
• Current registration to practice Occupational Therapy in New York State.

Responsibilities

• Assess, plan, develop and provide a home health plan of care for occupational therapy.
• Identify patient needs for other disciplines and services.
• Coordinate and evaluate the plan of care with all other disciplines, organizations and physicians for an assigned caseload.
• Communicate significant changes in patient status to the physician, rehab coordinator and the appropriate agency personnel when indicated.
• Demonstrates effective communication skills, keeping lines of communication open between Lead O.T. Supervisor, Team Managers and all staff involved in care of assigned patients.

Salary Range

USD $90,000.00 - USD $115, 000.00 /Yr.
