Job summary

Plaza West Healthcare & Rehab is seeking an Occupational Therapist to evaluate and treat residents with diverse needs, under physician orders, and to develop personalized care plans. You will guide families and caregivers in using adaptive equipment and maintain thorough patient records.

Role includes possible supervision of COTAs and rehab staff, adherence to privacy guidelines, and completion of required regulatory forms. Licensure and preferred clinical experience are emphasized.

Qualifications

• Bachelor or Master of Science in Occupational Therapy required.
• Current license in Occupational Therapy as mandated by state law.
• Minimum one year of clinical experience preferred.
• State-approved certification for advanced level treatments where applicable.

Responsibilities

• Under a physician’s orders, screens and evaluates resident/patient and develops an appropriate care plan.
• Instructs residents, families, and caregivers in the use of adaptive equipment such as wheelchairs, splints, and aids for eating and dressing.
• Maintains patient records in an accurate and timely manner.
• Devises special tools and treatments for the specific needs of patients.
• Completes required forms and documents in accordance with company policy and state and/or federal regulations.
• May supervise COTAs, Rehab Aides, or provisional licensed occupational therapists per state practice guidelines.

Education

Bachelor of Science in Occupational Therapy

Master of Science in Occupational Therapy

Current OT license

One year of clinical experience (preferred)

State-approved certification for advanced level treatments

• Participates in (or conducts) company-sponsored training to safeguard against improper us and disclosure of resident’s
• protected health information. Monitors and ensures compliance with company policies, procedures and state and federal law.
