Job summary

CareRite Centers, LLC is seeking a licensed Occupational Therapist to join The Bethany Center for Rehabilitation and Healing in a long-term care setting. You will assess patients, develop individualized treatment plans, and provide direct care under physician direction.

The role emphasizes accurate documentation, patient education, and collaboration with interdisciplinary teams to achieve patient-centered outcomes. Full-time and PRN shifts are available, with opportunities in Tennessee.

Qualifications

• Must have graduated from an accredited Occupational Therapist college or university.
• Must hold a valid professional license and registration for state of practice.
• Oversee and supervise Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care preferred.
• Strong documentation skills for patient records and reporting.
• Experience with evaluations, progress notes, re-certification and discharge documentation.
• Collaborates with all disciplines to provide patient-centered care.
• Demonstrates exceptional patient service and clinical excellence.
• Punctuality and professionalism in scheduled hours.
• Perform other duties as assigned by the Director of Rehabilitation.
• Compliance with confidentiality, HIPAA and residents' rights.

Responsibilities

• Assess referred patients requiring rehabilitation services and determine functional goals.
• Establish and administer treatment programs with goals tailored to patient capacity.
• Document patients' condition and educate patients and families in therapy methods.
• Supervise OT Assistants and collaborate with all disciplines to provide best patient care.

Occupational Therapist

The Bethany Center for Rehabilitation and Healing is looking for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patients condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Shift Needs: Full Time and PRN
