Job summary

Casa of Hobart is seeking a licensed Occupational Therapist to assess and treat patients in a rehabilitation setting. You will develop individualized treatment plans, set achievable goals, and document patient progress under physician direction.

Responsibilities include direct patient care, education for patients and families, regular evaluations and discharge planning, and collaboration with the multidisciplinary team to ensure high-quality, patient-centered care.

Qualifications

  • Must have graduated from an accredited Occupational Therapist college or university.
  • Must have a valid professional license and registration for state of practice.
  • Experience with long-term care and/or geriatric care is preferred but not required.

Responsibilities

  • Assess referred patients to determine rehabilitation needs.
  • Provide direct patient care to evaluate medical condition and functional abilities.
  • Establish and administer treatment plans with goals per physician direction.
  • Set realistic goals tailored to patient capacity and tolerance.
  • Document patient condition and progress through records and notes.
  • Educate patients and families on therapy methods and home programs.
  • Conduct evaluations, progress notes, re-certifications and discharge documentation.
  • Collaborate with all disciplines to ensure patient-centered care.
  • Perform other duties as assigned by the Director of Rehabilitation.

  • Educate patients and families in an appropriate therapy method.
  • Evaluations, weekly progress notes, re-certification and discharge documentations
  • Collaborates with all disciplines to provide best patient centered treatment and care
  • Perform all other duties assigned by Director of Rehabilitation
  • Compliance with confidentiality, HIPPA and Federal Residents Rights
