Job summary

AristaCare is seeking an Occupational Therapist for weekend per diem coverage in New Jersey. You will provide therapy services to residents in accordance with standards and company procedures, under the direction of the administrator and regional support team.

Responsibilities include evaluating residents, developing treatment plans with short and long-term goals, documenting per CMS and NetHealth EMR, and communicating findings to families and the interdisciplinary team.

Qualifications

• Bachelor of Science in Occupational Therapy from an accredited program.
• Active New Jersey Occupational Therapy license.
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Provide Occupational Therapist services to residents per standards and procedures.
• Adhere to documentation and billing procedures.
• Demonstrate sound judgment in evaluation, planning, implementation of therapy programs.
• Screen residents regularly to determine need for intervention.
• Screen referrals to therapy to determine need for treatment.
• Evaluate residents to obtain data for treatment planning.
• Develop treatment plans with short and long-term goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families and team.
• Complete CMS-compliant documentation in NetHealth EMR.

Available Shift: Weekend Per Diem

Job Purpose:

To assist in planning, organizing, developing, and directingOccupational Therapistservices in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by DOR, Administrator, or regional support team to assure the highest degree of quality resident care is always maintained.

Job Functions:

• Provide Occupational Therapistservices to residents in accordance with established standards of practice, Company procedures, Therapy Department procedures and productivity standards.

• Adhere to Company procedures regarding documentation and billing of Occupational Therapistservices.

• Demonstrate sound judgment in the evaluation, planning, implementation, and follow‑up of resident therapy programs.

• Conduct screening of residents at regular intervals to determine the need for intervention/treatment.

• Conduct timely screening of residents referred to physical therapy to determine the need for intervention/ treatment.

• Evaluate residents to obtain data necessary for treatment planning and implementation.

• Conduct specialized evaluations as indicated.

• Develop treatment plans by establishing short and long‑term goals and methods to achieve identified goals.

• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/ responsible parties, and interdisciplinary team members.

• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems

Education & Requirements:

• Minimum of Bachelor of Science Degree in Occupational Therapyfrom an accredited program

• Must have an active Occupational Therapy license in New Jersey

• Ongoing continuing education and professional development to maintain current licensure and certification -
