Job summary

The Palace Care Center in Maple Shade, New Jersey is seeking a compassionate Occupational Therapist (OT) to help residents regain independence in daily activities and mobility in a sub-acute nursing home setting.

You will collaborate with an interdisciplinary team, develop individualized treatment plans, and educate residents and caregivers on adaptive techniques and equipment use, while ensuring HIPAA compliance and safety.

Responsibilities

• Evaluate and assess residents' physical, cognitive, and psychosocial abilities to identify deficits and goals.
• Plan and implement evidence-based interventions to improve ADLs, mobility, and functional independence.
• Provide therapeutic exercises, manual therapy, and neuromuscular re-education.
• Educate residents and caregivers on adaptive techniques and equipment use.
• Collaborate with nurses, physicians, therapists, and social workers to coordinate care and document progress.
• Maintain HIPAA-compliant, accurate documentation and ensure safety and infection-control standards.

Job Summary:

The Palace Care Center is seeking a compassionate and skilled Occupational Therapist (OT) to join our team in Maple Shade, New Jersey. As an OT, you will provide essential therapy services to patients in a sub-acute nursing home setting, helping them regain functional independence in daily activities, improve mobility, and enhance their overall quality of life. Occupational Therapist (OT) will work collaboratively with an interdisciplinary team to ensure comprehensive patient care.

Key Responsibilities:

• Patient Evaluation & Assessment:
  • Conduct comprehensive assessments of residents' physical, cognitive, and psychosocial abilities.
  • Identify functional deficits and develop individualized treatment goals.
  • Assess the need for adaptive equipment and modifications.
• Treatment Planning & Implementation:
  • Develop and implement evidence-based therapy interventions to improve ADLs (Activities of Daily Living), mobility, and overall functional independence.
  • Provide therapeutic exercises, manual therapy, and neuromuscular re-education techniques.
  • Educate residents and caregivers on adaptive techniques and equipment use.
  • Modify treatment plans based on patient progress.
• Collaboration & Communication:
  • Work closely with nurses, physicians, physical therapists, speech therapists, and social workers to provide holistic care.
  • Participate in care plan meetings and family education sessions.
  • Maintain clear and timely documentation of patient progress, interventions, and outcomes.
• Compliance & Safety:
  • Ensure all therapy services comply with federal, state, and facility regulations.
  • Adhere to infection control protocols and patient safety standards.
  • Maintain confidentiality of patient records in accordance with HIPAA regulations.
• Equipment & Resource Management:
  • Recommend and assist with acquiring adaptive equipment (e.g., wheelchairs, splints, assistive devices).
  • Train staff on proper use of therapy-related equipment.
