Job summary

Phoenix Home Care & Hospice is seeking an Occupational Therapist to support patients in the Jefferson City and Columbia areas. This full-time, days position offers a $5,000 sign-on bonus and a pay range of $85,000–$95,000 per year.

You will provide home-based OT services, evaluate needs, and develop individualized plans to improve daily living skills and independence, while collaborating with the care team and documenting progress.

Qualifications

• Active Occupational Therapist license in Missouri.
• Home health experience preferred but not required.
• Strong clinical judgment and time management skills.
• Ability to work independently and manage your schedule.
• Strong communication and patient education skills.

Responsibilities

• Provide occupational therapy services in the home based on the physician plan of care.
• Evaluate patient needs and develop individualized treatment plans.
• Focus on improving daily living skills, safety, and independence.
• Collaborate with the care team to ensure quality outcomes.
• Educate patients and families on techniques and care plans.
