Phoenix Home Care is seeking a dedicated Occupational Therapist to support patients in the Jefferson City and Columbia, MO areas, delivering in-home care and helping patients regain independence in their own homes.

This full-time, days position offers a $5,000 sign-on bonus and competitive pay. You will work with a collaborative team to provide high-quality home health services and meaningful patient progress.

Qualifications

• Active Occupational Therapist license in Missouri.
• Home health experience preferred but not required.

Responsibilities

• Provide occupational therapy services in the home based on the physician plan of care.
• Evaluate patient needs and develop individualized treatment plans.
• Focus on improving daily living skills, safety, and independence.

What You Need
