Choice Health at Home in Fort Collins, CO seeks a compassionate Occupational Therapist (OT) for part-time home health care. You will provide one-on-one patient care in patients' homes, assess needs, and help improve daily living skills and safety.

The ideal candidate is a licensed OT with strong communication and documentation skills and a passion for enabling independence. Home health experience is a plus, and flexibility is valued in this role.
