Job summary

Fairfax Nursing Center is seeking a PRN Occupational Therapist to assess and treat patients in need of rehabilitation services. You will work under the direction of the physician to develop and manage individualized treatment programs and to document patient progress.

You will collaborate with the care team, educate patients and families, and ensure compliance with confidentiality and resident rights regulations. Previous long-term care or geriatric experience is preferred.

Qualifications

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not limited to.

Responsibilities

• Assess referred patients to determine rehabilitation needs.
• Provide direct patient care to evaluate medical condition, functional abilities, limitations and rehab potential.
• Establish and implement a treatment program with goals aligned to patient capacity under physician direction.
• Set realistic, achievable goals for each patient.
• Document and record patient status and progress.
• Educate patients and families on therapy methods.
• Complete evaluations, weekly progress notes, re-certifications and discharge documentation.
• Collaborate with all disciplines to deliver patient-centered care.
• Perform additional duties as directed by the Director of Rehabilitation.
• Maintain confidentiality and comply with HIPAA and federal resident rights.

Duties and Responsibilities:

• Responsible for the assessment of referred patients that require rehabilitation services

• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.

• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.

• Set realistic and achievable goals for their patients

• Document and record the patients’ condition

• Educate patients and families in an appropriate therapy method.

• Evaluations, weekly progress notes, re-certification and discharge documentations.

• Collaborates with all disciplines to provide best patient centered treatment and care.

• Perform all other duties assigned by Director of Rehabilitation

• Compliance with confidentiality, HIPPA and Federal Residents Rights.
