Job summary

Apploi in Florida seeks a licensed Certified Occupational Therapist to join our Skilled Nursing Facility rehabilitation team on a per diem basis. The OT evaluates residents, develops individualized treatment plans, and provides therapy to promote independence, safety, and quality of life.

The role emphasizes ADLs, functional mobility, and geriatric rehab, with supervision of COTAs, adherence to Medicare guidelines, and thorough documentation.

Qualifications

• Master's or Doctorate in Occupational Therapy from an accredited program.
• Current OT license in Florida in good standing.
• Experience in skilled nursing or long-term care preferred.
• Knowledge of geriatric rehab and Medicare regulations.
• Strong communication and documentation skills.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans.
• Provide therapy focused on ADLs, functional mobility, strength, coordination, cognition, and safety.
• Monitor resident progress and adjust treatment plans as needed.
• Complete accurate and timely documentation in compliance with Medicare and facility guidelines.
• Participate in interdisciplinary care planning and discharge planning.

• Educate residents and families on adaptive techniques and equipment.
• Maintain compliance with federal, state, and facility regulations.

We are seeking a compassionate and licensed Certified Occupational Therapist (OT) to join our Skilled Nursing Facility rehabilitation team.
