Job summary

Evolve Therapy Services is seeking an Occupational Therapist for an in-house Skilled Nursing Facility located in Sutherland, Nebraska. This PRN opportunity focuses on delivering high-quality OT services within a collaborative care team.

Applicants must hold an OT degree and an active license in the state, with ongoing commitment to licensure maintenance and professional development. The role includes supervision of COTA and coordination with the interdisciplinary team to ensure optimal resident

Qualifications

• Must hold a degree in Occupational Therapy and an active state license.
• Commitment to ongoing continuing education to maintain licensure.
• Ability to supervise and collaborate with COTA as part of the interdisciplinary team.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Evaluate residents and develop short- and long-term goals and plans.
• Communicate evaluation findings and treatment plans to residents, families, and team.
• Schedule resident therapy in coordination with team members.
• Provide OT therapy per plan and document and bill accordingly.
• Perform other duties to effectively treat patients.
• Oversee COTA and ensure documentation and appropriate reimbursement practices.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
