Job summary

Evolve Therapy Services is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in Sutherland, NE. This PRN position supports clinical excellence and resident-centered care.

You will plan, organize, and direct OT services, collaborate with the interdisciplinary team, supervise COTA, and ensure proper documentation and billing.

Active OT license in Nebraska is required; ongoing CEU participation is encouraged; competitive pay and growth opportunities are available.

Qualifications

• Must have active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best intervention.
• Evaluate residents to inform treatment plans; establish short- and long-term goals and methods to achieve goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individualized therapy schedules in coordination with other team members.
• Conduct therapy according to established treatment plans and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned to effectively treat patients.
• Oversee COTA as needed and ensure documentation and treatments are carried out with clinical excellence and proper reimbursement practices.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient

Education & Qualifications:
