Qualifications

• Licensed Occupational Therapist in good standing.
• Open to applicants of all experience levels.

Responsibilities

• Screen and evaluate patients with cognitive/ADL functional disorders and create care plans per standards.
• Use standardized assessments and evidence-based practices to guide interventions.
• Provide rehabilitative, skilled, and medically necessary treatments.
• Perform ADL retraining, therapeutic activities, exercises, modalities, and device usage.
• Assess progress and adjust treatments in the clinical record.
• Evaluate outcomes and refer as needed based on progress.
• Deliver counseling to patients, families, and caregivers about physical disorders.
• Communicate progress, barriers, and plans with the supervisor and health team.
• Prepare comprehensive discharge summaries per regulatory requirements.
