Job summary

Astoria Health & Rehab Center seeks an Occupational Therapist to join our skilled nursing facility therapy team. The primary function is to restore independence by improving physical function and guiding patients toward their therapy goals.

You will educate patients on ongoing self-management, work with a multidisciplinary team, and ensure effective discharge planning. Full-time and part-time options with days shifts available.

Qualifications

• OT degree from an accredited program (Bachelor's or Master's).
• Current license to practice occupational therapy in the state.
• Basic computer skills with EHR and service delivery software.

Responsibilities

• Return patients to optimal functional levels within a SNF setting.
• Educate patients on maintaining function after discharge with home programs.
• Support therapy goals in collaboration with physicians and team.
