Job summary

Evolve Therapy Services is seeking a compassionate Occupational Therapist for an in-house Skilled Nursing Facility partner in upstate New York. This full-time opportunity may include relocation options, plus part-time or PRN positions.

The OT will plan, organize, and direct therapy services, conduct resident screenings, develop treatment plans, and collaborate with the care team to ensure high-quality resident care while ensuring proper documentation and billing.

Qualifications

• Must hold an active Occupational Therapy license in the practicing state(s).
• Ongoing continuing education and professional development to maintain current licensure and certification.
• Degree in Occupational Therapy.

Responsibilities

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education

Degree in Occupational Therapy

Active OT license in practicing state(s)
