Job summary

Evolve Therapy Services in Key West, FL is seeking an Occupational Therapist to lead in-house therapy at our Skilled Nursing Facility. This full‑time role offers a salaried schedule with a 4x10s option noted in the posting.

You will screen residents, develop treatment plans, set goals, and coordinate with an interdisciplinary team. Supervise COTA and ensure proper documentation and billing in line with current standards; active FL OT license required.

Qualifications

• Degree in Occupational Therapy is required.
• Active Occupational Therapy license in Florida or practicing state(s).
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Conduct regular screening of residents to determine best interventions.
• Evaluate residents to inform and develop treatment plans and goals.
• Interpret and communicate evaluation findings and recommendations to residents and interdisciplinary team.
• Establish individual resident therapy schedules with team members.
• Conduct therapy according to treatment plans and ensure proper documentation and billing.
• Oversee COTA as needed and ensure documentation and treatments meet reimbursement and clinical standards.

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

Full Time (Salaried or Flexible Scheduling/4x10s available), Part Time, or PRN

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
