Lock Haven Rehabilitation & Senior Center in Lock Haven, PA, is seeking a full-time Occupational Therapist to join our in-house rehabilitation team within a Skilled Nursing Facility. Qualifications

• Current Pennsylvania Occupational Therapist license or eligibility for licensure.
• Strong clinical, communication, and documentation skills.

• Ability to work effectively within a multidisciplinary team.
• SNF/LTC experience preferred, but not required.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled occupational therapy services focused on improving independence with daily living activities.
• Collaborate with physicians, nursing staff, and rehabilitation team members.
• Complete timely and accurate documentation.
• Educate residents, families, and caregivers on treatment goals and progress.
• Ensure compliance with facility, state, and federal regulations.
