Job summary

Covenant Case Management Services is seeking an enthusiastic Occupational Therapist to join our team. The OT provides consultative services to individuals with IDD enrolled in the NC Innovations Waiver, assessing functional skills and environmental needs, and delivering training to caregivers, DSPs, and team members to promote independence and safety.

Qualifications

• Experience assessing activities of daily living (ADLs), fine motor skills, sensory needs, self-regulation, and environmental supports.
• Ability to develop and update consultative support plans aligned with Person-Centered Plans (PCP).
• Experience training caregivers, Direct Support Professionals (DSPs), and support staff on adaptive strategies and equipment use.
• Knowledge of adaptive devices, sensory supports, and environmental modifications to promote independence and safety.
• Experience documenting assessments and training in compliance with program requirements.

Responsibilities

• Conduct ADL, fine motor, sensory, and environmental assessments.
• Develop consultative support plans aligned with PCP.
• Train caregivers and DSPs on adaptive strategies and equipment use.
• Provide recommendations for adaptive devices and environmental modifications.
• Collaborate with interdisciplinary team (PT, Speech, Nursing, Behavioral Specialists, Case Managers).
• Document assessments and training per NC Innovations Waiver and agency requirements.
• Participate in treatment team meetings and care plan reviews.
• Stay current on evidence-based OT practices and IDD regulations.
