Your Health in Brunswick, GA seeks a skilled Occupational Therapist to service patients in homes and facilities. You will perform comprehensive assessments, design customized treatment plans, and supervise COTAs delivering therapy.

This salary-based role includes monthly bonuses and a standard 8-hour workday (Mon-Fri). You will collaborate with care teams and regional leadership to ensure compliant, high-quality care.

Qualifications

• Master’s or Doctoral degree in Occupational Therapy from an accredited program.
• Current state license to practice as an Occupational Therapist.
• Minimum of two (2) years of clinical experience, preferably in home health or outpatient.
• Proficiency in Telehealth platforms and EHR.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.

Responsabilités

• Conduct comprehensive evaluations of patients’ physical, emotional, and social conditions.
• Identify patient needs and set realistic goals for therapy.
• Develop individualized treatment plans aimed at improving daily living and working skills.
• Work with COTAs, providing guidance and supervision; review implementation of plans.
• Use telehealth tools to collaborate with COTAs and other professionals.
• Facilitate daily visits with providers to ensure coordination of care and alignment on plans.
