Job summary

Nightingale Home Healthcare of Minnesota Inc in Minnetonka, MN is seeking a part-time Occupational Therapist to provide in-home therapy, evaluate patients, design treatment plans, and coordinate care with the rehab team.

You will work with limited travel, receive per-visit pay starting at $75, flexible scheduling, mileage reimbursement, and opportunities for merit increases. Requires MN OT license, CPR, and reliable transportation. HIPAA compliance and one-on-one patient care are essential.

Qualifications

• Licensed OT in MN required.
• Minimum 1 year OT experience preferred.
• Experience with OASIS and WellSky (Kinnser) strongly preferred.
• Current TB test within last 12 months and CPR certification.
• Valid driver’s license and reliable transportation.
• Ability to attend a 2–3 day orientation at Minnetonka office.

Responsibilities

• Provide effective patient care using approved therapy techniques and procedures.
• Evaluate patients’ condition, function level, and treatment goals.
• Develop individualized treatment plans and update as needed.
• Supervise COTAs and HHAs per care plan requirements.
• Document treatment notes and progress in the patient’s chart daily.
• Assist patients and families with home exercise programs and adaptive equipment needs.
• Recommend and evaluate adaptive equipment for the home setting.
• Maintain compliance with HIPAA regulations and company policies.
• Coordinate total patient care with the healthcare team.
• Complete discharge summaries and obtain orders when needed.
• Submit patient documentation and visit records in a timely manner.

Position Summary:
