Job description

Summary

The Occupational Therapist in Home Health is responsible for assessing and evaluating patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, adaptive equipment, and other occupational therapy needs as defined by medical conditions. Based on this assessment, the Occupational Therapist helps determine a treatment plan, performs interventions to improve the patient's well-being, and evaluates progress.

Per Visit Rate: $59 - $60 for Regular Visits / $74 for Add-On Visits

Responsibilities

  1. Provides services within the scope of practice as defined by state laws, in accordance with the plan of care, using evidence-based techniques, and in coordination with the healthcare team.
  2. Evaluates the patient's functional status, body systems as required for CMS documentation, and occupational therapy needs. Consults with physicians for developing the therapy plan of care.
  3. Observes, records, and reports patient responses and condition changes to the nurse supervisor and physician.
  4. Instructs and informs the healthcare team, patient, and family/caregivers about the plan of care, limitations, and progress.
  5. Visits patients within the service area and nearby agencies as needed.
  6. Completes progress notes for each visit and submits them per policy.
  7. Completes evaluations and develops the OT plan of care within state guidelines. Reports outcomes and projected care frequency within 24 hours.
  8. Participates in staff meetings and committees as requested.
  9. Supervises home health aides if permitted by state regulation.
  10. Supervises OTA as per policy and regulations. Prepares discharge summaries every 60 days.
  11. Conducts patient assessments evaluating functional ability.
  12. Provides in-service education as requested.
  13. Adheres to therapy standards of practice.
  14. Maintains current clinical knowledge and licensure.
  15. Flexible schedule required, including weekend rotation and observed holidays.

Education and Experience

  • Current Occupational Therapy licensure in Colorado.
  • Current CPR certification.
  • Valid driver's license, vehicle insurance, and access to transportation.
