Job summary

Firelands Health is seeking an Occupational Therapist to join our Home Health team serving Sandusky and surrounding areas. You will evaluate, plan, and implement patient-centered therapy to improve functional independence and quality of life.

You will work with an interdisciplinary team, document clearly, and adapt treatment plans to each patient’s goals while complying with regulatory standards. ACOTE-accredited degree and Ohio licensure are required.

Qualifications

  • Bachelor's or Master's degree from ACOTE-accredited program.
  • State licensure in Ohio or license eligible; registration through professional association.
  • CPR Certification.
  • Previous clinical rehabilitation experience preferred.
  • Ability to move, transfer and assist patients; effective communication.
  • Valid Ohio driver's license and insurability under Firelands' policy.
  • Punctual attendance at work location.
  • Compliance with OSHA and CDC guidelines and PPE use.
  • Successful completion of a 90-day probationary period.

Responsibilities

  • Completes patient evaluations and selects appropriate tools and goals.
  • Plans and provides treatment aligned with established standards of care.
  • Documents clearly to communicate patient status and needs.
  • Teaches patients skills for participation and evaluates progress.
  • Participates in training of medical and nursing personnel in OT techniques.
  • Designs and fits adaptive devices such as splints and braces.
  • Collaborates with rehabilitation team to tailor activity programs.
  • Requisitions supplies and maintains equipment; supports performance improvement.
  • Upholds ACE service excellence and adheres to safety protocols.
  • Performs other duties as assigned.

What you will do:
  • Completes evaluations of assigned patients in accordance with the established standards of care and practice. Independently selects appropriate evaluation tools consistent with patient’s history, diagnosis, and medical status, per documentation and observation. Evaluation techniques are performed safely and accurately, taking into account the patient’s pre-morbid history and current physical psychosocial, and educational status/needs. Long and short term goals are realistic, measurable, and address functional needs. Goals are written upon completion of evaluation.
  • Plans and provides treatment to assigned patients as appropriate in accordance with established standards of care and practice. Demonstrates knowledge about equipment/materials utilized in patient care per observation and chart documentation. Modifies short- and long-term goals, activities, frequency and/or length of treatment in response to patient’s status in collaboration with team members.
  • Completes all documentation. Responsible to independently write clear, concise, and relevant documentation, and to include documentation that facilitates communication of the patient’s status and ongoing needs.
  • Teaches patients the skills and techniques required for participation in activities; and evaluates patients’ progress, attitudes, and behavior as relates to their rehabilitative potential. Adapts programs according to the needs and abilities of individual patients.
  • Conducts training programs and participates in training medical and nursing personnel in occupational therapy techniques and objectives.
  • Designs and fits adaptive devices such as splints and braces for patient use.
  • Regularly demonstrates the ability to serve adult patients in the routine performance of duties.
  • Displays positive guest relations in the performance of duties.
  • Consults with other members of the rehabilitation team to select the most appropriate activity program consistent with the needs and capabilities of each patient.
  • Requisitions supplies and equipment.
  • Cleans and repairs tools and equipment as needed.
  • Participates in department performance improvement activities as assigned and attends required training.
  • Demonstrates service excellence and ACE.
  • Performs other duties as assigned.
What you will need:
  • Bachelor's or Master’s degree from a program jointly accredited by the ACOTE or a graduate of the ACOTE accredited career mobility program.
  • Licensure in the state of Ohio or license eligible. Registration nationally through the professional association.
  • Previous clinical experience/rehabilitation experience preferred.
  • CPR Certification.
  • Demonstrated ability to keep abreast of new research and practices in the field so as to provide the patients the best possible care and treatment.
  • Ability to move, transfer and assist average size patient and be mobile.
  • Ability to understand and effectively communicate in both written and oral form.
  • Must possess a valid State of Ohio motor vehicle operator's license and be insurable under Firelands' auto policy. Employee must provide proof of personal automobile liability insurance coverage upon request.
  • Punctual attendance at assigned work location is required.
  • Ability to work in a safe and efficient manner and maintain an accident free work place, including ability to demonstrate a working knowledge of emergency codes.
  • Ability to comply with OSHA regulations and CDC standard and transmission-based precaution recommendations and to utilize proper personal protective equipment. Ability to comply with provisions of applicable S.D.S. forms.
  • Successful completion of a ninety (90) day probationary period.
