Job description

BAYADA Home Health Care

is currently seeking an experienced Occupational Therapist, OT, for a Per Diem opportunity performing Home Health visits in senior living communities throughout Oxford and surrounding areas of Chester County.

As a home care Occupational Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, enabling them to receive the medical care required to remain at home.

Occupational Therapist, OT Responsibilities:
  1. Make home visits to clients in designated geographic territories.
  2. Perform thorough evaluations of patients' functional status, including muscle function, endurance, visual coordination, communication skills, and ADL and IADL performance.
  3. Assess cognitive, psychological, and motor aspects of performance and develop individualized intervention plans to achieve favorable outcomes.
  4. Evaluate home environment for hazards or barriers to independent living.
  5. Identify equipment needs and environmental adaptations to enhance client outcomes.
  6. Continuously assess and revise the occupational therapy care plan, collaborating with nursing staff as appropriate.
  7. Educate and instruct patients, family members, or other representatives in rehabilitative care and activities to promote health, safety, and independence.
  8. Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet.
Occupational Therapist, OT Qualifications:
  1. Current Pennsylvania Occupational Therapist license.
  2. Prior home care experience preferred but not required.
  3. Minimum of one year of recent work experience as an Occupational Therapist.
  4. Graduation from an accredited occupational therapy program recognized by the AMA and AOTA.
  5. Ability to work independently and manage time effectively.
  6. Strong interpersonal, organizational, and problem-solving skills.
  7. Solid computer skills; EMR experience preferred.
