  • Provides specialized occupational therapy services to primarily adult and geriatric patients in their homes.
  • Conducts comprehensive patient assessments and develops individualized treatment plans.
  • Implements therapeutic interventions and provides home safety recommendations to improve quality of life and independence.
  • Consults and coordinates with home health interdisciplinary team regarding the plan of care.
