Job summary

Adventist Health is seeking a Home Health Occupational Therapist to deliver personalized therapy in patients' homes within our home health and hospice communities.

You will evaluate patients, develop treatment plans, and provide interventions to improve daily living, cognition, and mobility while coordinating with the care team.

A state OT license, an OT/OTD degree, BLS/CPR, and a valid driver’s license are required; hospital-grade patient-centered care is our standard.

Qualifications

• Licensed OT in applicable state(s).
• Master’s degree in OT or Doctor of OT (OTD) required.
• BLS/CPR certification required.
• Valid driver’s license and reliable transportation.
• Strong communication, organization and patient care skills.

Responsibilities
