Job description

Job Overview

St. Luke's is inviting applications for a Full‑Time Physical Therapist to provide Home Health services in the Stroudsburg area. The therapist will collaborate with the interdisciplinary team to deliver high‑quality physical therapy that follows physicians’ orders and the agency’s policies, working in patients’ homes.

Responsibilities
  • Perform initial evaluations, identify functional deficits, and establish treatment plans with short and long‑term goals.
  • Treat patients to alleviate pain, restore or enhance function, and maintain optimal performance through therapeutic exercises, massage, and various physical modalities.
  • Guide patients through active and passive exercises, muscle re‑education, gait training, and functional daily‑living activities, including transfers and prosthetic training.
  • Use modalities such as low‑voltage generators, ultrasound, heat, and cold as needed.
  • Coordinate outpatient therapy services for procedures not completed at home.
  • Observe, record, and report patient responses and any changes in condition to the treating physician.
  • Communicate with the Rehab Program Manager, Patient Care Manager, and Clinical Coordinator to plan total patient and family care.
  • Consult with other disciplines as required; maintain a family‑oriented documentation approach.
  • Document all care in a timely, accurate, legible manner in compliance with agency policy.
  • Evaluate outcomes, revise treatment plans, and plan discharge as appropriate.
  • Supervise Physical Therapy Assistants and instruct other health‑team personnel and family members when appropriate.
  • Educate patients on the use of wheelchairs, braces, crutches, walkers, canes, prosthetic and orthotic devices.
  • Facilitate equipment provision and provide orientation to PT students and new hires on scope of practice and home‑care skills.
  • Participate in patient‑centered unit meetings.
  • Use personal vehicle to travel to patient residences; maintain valid PA or NJ driver’s license, good driving record, and required insurance.
Physical and Sensory Requirements
  • Ability to sit for up to 90 minutes.
  • Ability to stand for 30–45 minutes consecutively.
  • Use of hands/fingers to manipulate machine controls.
  • Lifting/moving patients up to 300 lbs; pushing/pulling patients weighing up to 300 lbs.
  • Frequent stooping, bending, squatting, crouching, kneeling, and reaching above shoulder level.
  • Tactile sensation for muscle spasms and skin temperature.
  • Normal hearing for conversation and call bell response.
  • Normal vision with depth and peripheral perception.
Qualifications
  • Doctoral or Master’s degree from an accredited physical therapy program.
  • Licensed by the Pennsylvania State Board of Physical Therapy.
  • One to two years of experience under a qualified registered Physical Therapy Supervisor (preferred).
  • CPR certification required.
  • Valid PA or NJ driver’s license with a clean driving record and appropriate vehicle insurance; must maintain vehicle and obey state regulations.
