Job summary

Hebrew SeniorLife in Dedham, MA is seeking an experienced occupational therapist to implement care standards and lead the plan of care with physicians. You will conduct home visits, evaluate functional status, adapt plans, and communicate with PCPs, families, and the care team.

You will educate patients, train in daily activities and use prosthetic/orthotic devices, while maintaining accurate EMR documentation and coordinating with other disciplines for integrated care.

Qualifications

• A current license as an Occupational Therapist in Massachusetts.
• Minimum of one year of recent OT work experience preferred.
• Graduate of an OT program accredited by AMA/AOTA with transcript or diploma.
• Strong clinical, oral and written communication skills.
• CPR certification current; valid driver's license and insured vehicle.
• Ability to work independently; strong organization and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Makes home visits to patients in designated geographic territories.
• Performs thorough evaluations of functional status and ADL/IADL performance.
• Evaluates home environment for hazards and equipment needs.
• Revises the OT care plan with the multidisciplinary team.
• Develops treatment plans and communicates the plan to PCP/referring physician.

• Maintains clinical records and reports changes to team physicians.
• Completes OASIS documentation within 24-48 hours consistently.
• Maintains coordination with other disciplines to ensure patient care.
• Participates in team meetings, case conferences, and in-services.
• Identifies needs for other home health services and refers appropriately.
• Instructs family/caregivers and other personnel in treatment regime when indicated.
• Keeps professional knowledge up to date through continuing education.

Position Summary :

The occupational therapist is responsible for the implementation of standards of care for occupational therapy services and for adherence to the plan of care established by the physician.

Position Responsibilities Include:

• Makes home visits to patients in designated geographic territories.

• Performs thorough evaluations of patients’ functional status, including muscle function, endurance, visual coordination, written and verbal communication skills and ADL and IADL performance.

• Evaluates the home environment for hazards or barriers to more independent living and identifies equipment needs.

• Continually assess and revise the occupational therapy care plan, and participate with other team members in the multidisciplinary care plan as appropriate.

• Develops treatment program and establishes goals for improved function. Communicates plan of care to PCP or referring physician.

• Teach new skills or retrain patient in once familiar daily activities that have been lost due to illness or injury.

• Train patient in the use of prosthetic and/or orthotic devices.

• Maintains appropriate clinical records, clinical notes, and reports to the physician/appropriate team member any changes in the patient’s condition.

• Complete OASIS documentation with accuracy and submits to the office within 24-48 hours on a consistent basis.

• Maintains contact/communication with other disciplines involved in the patient’s care to promote coordinated efficient care.

• Meets with rehabilitation manager on a regular basis to facilitate patient’s coordinator of care.

• Attends and contributes to in-services, case conferences, and team meetings as required by the agency to ensure coordinated and comprehensive plans of care for the patients.

• Identifies patient and family/caregiver needs for other home health services and refers as appropriate.

• Instructs patient’s family/caregiver and other organization health care personnel in patient’s treatment regime when indicated and appropriate.

• Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education.

• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.

• Promotes and maintains an agency environment that is in compliance with federal, state, and local regulatory agencies.

• Participates in personal and professional growth and development including staff meetings and in-service education.

• Communicates with patients, families, and other health professionals in a manner that conveys respect, caring, and sensitivity.

• Communicates with the physician and/or nurse practitioner regarding patient status and medical intervention.

Qualifications Include:

• A current license as an Occupational Therapist in Massachusetts

• A minimum of one year of recent work experience as an Occupational Therapist is preferred.

• A graduate of a occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA, as indicated by school transcript or Diploma

• Has strong clinical skills as well as good oral and written communication

• Possesses and maintains current CPR Certification

• Must be a licensed driver with an automobile that is insured in accordance with state and/ or organization requirements and is in good working order, or other reliable means of transportation.

• Ability to work independently and manage time effectively

• Possesses strong interpersonal skills, organizational skills and problem solving skills

• Solid computer skills; prior experience with electronic medical records preferred.
