Job summary

Partners In Home Care (PIHC) in Western Montana seeks a compassionate Occupational Therapist to join our Home Health team serving Ravalli County. You’ll conduct in-home evaluations and develop individualized treatment plans, delivering one-on-one care in patients' homes across the Bitterroot Valley.

Qualifications

• Bachelor’s degree or higher in Occupational Therapy.
• Current Montana OT license or ability to obtain.
• Two+ years of OT experience preferred.
• Home health or clinical rehab experience a plus.

Responsibilities

• Conduct comprehensive in-home OT evaluations and develop individualized treatment plans.
• Provide skilled, evidence-based OT interventions and adjust plans as patient needs evolve.
• Complete and score OASIS assessments accurately and in compliance with Medicare regulations.
• Maintain knowledge of payer guidelines and Medicare Conditions of Participation.
• Participate in interdisciplinary case conferences and QAPI initiatives.
• Supervise OTAs and provide oversight to CNAs when applicable.
• Manage patient care episodes including admissions, reassessments, and discharges.
• Document services and communications using point-of-care electronic systems.
• Coordinate daily schedules to support continuity of care and efficient travel.
• Maintain Montana OT licensure and continuing education; uphold professional standards.
• Engage in agency training and professional development activities.

Job description

Partners In Home Care (PIHC), Western Montana's leading non-profit home health provider since 1987, is seeking a compassionate Occupational Therapist to join our growing Home Health team serving Ravalli County.

This position offers the opportunity to build meaningful relationships with patients while delivering personalized, one-on-one care in the comfort of their homes across the Bitterroot Valley, one of Montana's most scenic communities.

Department: Home Health
Employment Type: Full-Time or Part-Time
Schedule: Monday–Friday, with flexibility based on FTE and patient care needs.

Location: Based out of Hamilton, MT, serving patients throughout Ravalli County. Occasional travel within PIHC's broader service area may be required based on patient needs.

Essential Duties and Responsibilities

• Conduct comprehensive in-home occupational therapy evaluations and develop individualized treatment plans
• Provide skilled, evidence-based occupational therapy interventions and adjust plans as patient needs evolve
• Complete and score OASIS assessments accurately and in compliance with Medicare regulations
• Maintain knowledge of payer guidelines and Medicare Conditions of Participation

REQUIREMENTS

• Bachelor’s degree or higher from an accredited Occupational Therapy program
• Current Occupational Therapist license in the State of Montana (or ability to obtain)
• Valid driver’s license, reliable transportation, and appropriate auto insurance
• Access to high-speed internet
• Two or more years of occupational therapy experience preferred
• Home health or clinical rehabilitation experience a plus
