Job summary

The Chateau at Brooklyn Rehabilitation & Nursing Center in Brooklyn, NY is seeking an Occupational Therapist to join our on-site team. You will evaluate residents, develop and implement individualized treatment plans, and coordinate with the interdisciplinary team to support safe discharge and return to function.

New graduates are welcome; we provide structured mentorship, orientation, and ongoing clinical education from experienced clinicians who want to teach.

Qualifications

• Must hold an accredited Occupational Therapy degree.
• Active OT license and registration in the state of practice or license eligibility.
• Ability to supervise Occupational Therapy Assistants per state act.
• Comfort with clinical documentation including evaluations, progress notes, recertifications, and discharge summaries.
• Commitment to confidentiality and HIPAA regulations.

Responsibilities

• Evaluate referred residents to determine medical status, function, and rehabilitation potential.
• Build individualized treatment plans with achievable goals based on capacity and tolerance.
• Deliver skilled OT interventions focused on ADL performance, safety, and function restoration.
• Complete evaluations, progress notes, recertifications, and discharge documentation on time.
• Educate residents and families on therapy techniques and safety strategies.
• Collaborate with PT/SLP/nursing/dietitians to support discharge readiness.
• Recommend adaptive equipment and home/environment modifications.
• Supervise OTAs and students per state practice act.
