Job summary

The Chateau at Brooklyn Rehabilitation and Nursing Center in Brooklyn, NY is hiring an Occupational Therapist to join our interdisciplinary team in a thriving 189-bed facility. You will evaluate residents, develop goals, and deliver skilled OT focused on ADLs and safety.

We offer mentorship for new grads, structured orientation, and ongoing clinical education within our growing 35-facility network. Competitive pay of $43.00–$53.00 per hour, full-time schedule Monday to Friday, on-site work.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Valid Occupational Therapist license and registration in the state of practice, or license eligible.
• Ability to supervise Occupational Therapy Assistants in accordance with the state practice act.
• Comfort with clinical documentation including evaluations, weekly progress notes, recertifications, and discharge summaries.
• Commitment to confidentiality, HIPAA, and Federal Resident Rights.
• Experience in long term care or geriatric rehabilitation is preferred.
• New graduates and therapists new to skilled nursing are encouraged to apply.

Responsibilities

• Evaluate referred residents to determine medical status, functional ability, limitations, precautions, and rehabilitation potential
• Build individualized treatment plans with measurable, achievable goals based on each resident’s capacity and tolerance, under physician direction
• Deliver skilled occupational therapy interventions focused on ADL performance, safety, and return to prior level of function
• Complete evaluations, weekly progress notes, recertifications, and discharge documentation accurately and on time
• Educate residents, families, and caregivers on therapy techniques, safety, and carryover strategies
• Collaborate with PT, SLP, nursing, dietitians, and social work to deliver patient centered care and support discharge readiness
• Recommend adaptive equipment, durable medical equipment, and environmental modifications
• Supervise Occupational Therapy Assistants and students in accordance with the state practice act
