Continuum Rehab Group is seeking an Occupational Therapist to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities. We value a compassionate, professional culture and opportunities for growth within a supportive environment.

Qualifications

• Licensed Occupational Therapist (OT) in good standing.
• NBCOT designation if required by state.
• Degree in Occupational Therapy from accredited program (B.S., M.A., or M.S.).

Responsibilities

• Utilizes evidence-based practice to guide clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment as per regulatory and clinical standards.
• Delivers skilled OT services to residents in SNF/long-term care facilities.
• Provides counseling to patients and families about physical disorders and rehab goals.
• Supervises OT assistants, students, and support staff per licensure requirements.
