Continuum Therapy Partners seeks a licensed Occupational Therapist to join our growing national network. You will evaluate patients, develop individualized treatment plans to improve functional independence, and document progress.

NBCOT certification and an active state license are required, with 1 year of OT experience preferred. This full-time daytime role is based at Thief River Care Center in Thief River Falls, MN and includes a comprehensive benefits package, 401k with company match,

Qualifications

• Master's or Doctorate Degree in Occupational Therapy required
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required
• 1 year of previous experience as an Occupational Therapist preferred

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients' functional independence
• Documents patient progress and modifies treatment plans based on clinical outcomes
• Educates patients and families on adaptive equipment and home safety recommendations
