Job summary

Blue Stone Therapy in Wapello, IA is seeking an Occupational Therapist (OT) to provide rehabilitation services to adult and geriatric patients in a skilled nursing and post-acute setting. The OT will evaluate and plan care to improve ADLs, cognition, and mobility.

The OT will oversee therapy assistants, ensure timely documentation, and collaborate with an interdisciplinary team to optimize patient outcomes. A valid state license and degree in OT are required, with SNF experience preferred.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Occupational Therapist license in applicable state.
• Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred.
• Strong clinical reasoning, communication, and organizational skills.
• Commitment to patient-centered care and professional standards.

Responsibilities

• Perform occupational therapy evaluations and develop individualized plans of care.
• Provide skilled occupational therapy treatment focused on functional outcomes.
• Address ADLs, functional mobility, cognition, strength, positioning, and discharge planning.
• Supervise Occupational Therapy Assistants in accordance with regulatory and company standards.
• Maintain appropriate patient schedules per established guidelines.
• Complete timely, accurate documentation to support quality care and reimbursement requirements.
• Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes.
