Job summary

Bluestonetherapy is looking for an Occupational Therapist (OT) in Brookings, SD to provide therapy services in skilled nursing and post-acute rehabilitation settings. You will evaluate and treat adult and geriatric patients, developing individualized care plans to improve daily living activities and overall functional independence.

Qualifications

• Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred.
• Commitment to patient-centered care and professional standards.

Responsibilities

• Perform occupational therapy evaluations and develop individualized plans of care.
• Provide skilled occupational therapy treatment focused on functional and measurable outcomes.
• Address ADLs, functional mobility, cognition, strength, positioning, and discharge planning.
• Supervise Occupational Therapy Assistants in accordance with regulatory and company standards.
• Maintain appropriate patient schedules per established guidelines.
• Complete timely, accurate documentation to support quality care and reimbursement requirements.
• Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes.
