Job summary

Ghent Rehabilitation and Nursing Center in the United States seeks a licensed Occupational Therapist to evaluate and treat residents in a SNF setting. You will establish care plans, document progress, and collaborate with nursing, therapy and families to optimize resident function.

The position requires an OT degree, NYS license, and a driver’s license with a willingness to work in a dynamic, team-oriented environment. Knowledge of Medicare and regulatory guidelines is essential.

Qualifications

• Must have a current unencumbered NYS license as a Registered Occupational Therapist, Bachelor's degree or higher.
• Graduate of an accredited school of occupational therapy or international equivalent.
• Geriatric experience preferred.
• A current unrestricted driver's license and dependable transportation.
• Ability to communicate professionally with internal and external resources.
• Ability to function independently as well as part of a team.
• Ability to function effectively in stressful and changing situations.
• Ability to give and receive oral and written direction.

Responsibilities

• Screens residents for rehabilitation therapy needs.
• Completes Occupational Therapy evaluations upon physician order.
• Establishes OT Plan of Care per physician order.
• Provides OT plan of treatments and modalities.
• Modifies treatment program based on resident response.
• Documents resident progress toward OT goals.
• Discharges residents from Occupational Therapy services when appropriate.
• Recommends mobility, positioning, ROM and Self-care tasks to nursing.
• Completes Daily Activity Schedule, progress notes and re-certifications.
• Supervises Certified OT Assistants and unlicensed staff.
• Provides education to residents, staff and families.
• Demonstrates Medicare Part A PPS case management knowledge.
• Complies with Federal and State rehab regulations and standards.
• Maintains documentation to professional standards and regulatory requirements.
• Strives for productivity and self-initiates corrective action for non-attainment.
• Completes care plans and MDS as directed; assists in ordering equipment.
• Participates in discharge planning with interdisciplinary team.
